CAMP Forms
Below you will find the links to the two forms APEX Parkour Camp Health Form and Pick-Up Authorization Form. Please complete these forms, print them and bring them with you on the first day of camp. The forms will also be available when you arrive.
The Pick-Up Authorization Form give permission to people to pick up children from camp. Please put your name on the form as well (as the staff in the afternoon may not be the same as the staff in the morning). You may use one form for multiple children for which you are legal guardian or parent.
The Camp Health Form is to bring any medical or allergy concerns to our attention. Please fill out per child.
Also, please go to YOUR CHILD’S MINDBODY ACCOUNT and verify your contact information in the emergency contact field. If you cannot log in, contact us and we can reset your password or you can do this when you arrive at APEX.
